Free prescription pad template

Type your details below and the pad on the right updates as you go: your name and qualifications, PMDC number, clinic address, phone and timings. Then print as many as you need on A4 or A5, or save a PDF and take it to your printer for a proper pad.

It's free, there's nothing to sign up for, and what you type stays in your own browser.

Paper size

To get a PDF, choose Save as PDF as the printer. What you type stays in this browser; it isn't sent to us.

How to get a pad printed

Fill in your details and pick A4 or A5 (the half-page size most pads come in). Press Print or save as PDF. To print at the clinic, choose your printer; for a printing press, choose Save as PDF and send them the file. Most presses will turn a PDF into gummed pads of 100 sheets.

Your details are remembered in this browser, so next time you open the page they are already filled in.

What a prescription pad should carry

Your full name, qualifications and PMDC registration number, the clinic's name, address and phone, and room to write the patient's name, age and date. The left column on this template has space for complaint, diagnosis, blood pressure, pulse, temperature and weight, which is what most GPs note before writing.

Under the Punjab Drugs Rules a prescription has to be written, signed and dated, and show the patient's name and address, the quantity and the dose, so leave yourself room for those.

Or skip the pad altogether

In FindClinica you pick medicines from your own list with their usual dose, and the finished prescription goes to the patient on WhatsApp or to the printer, already filled in and kept on the patient's file. How prescriptions work in FindClinica.

Common questions

Can I download it as a Word file?

No, it saves as a PDF. Every printer and printing press can print a PDF, and it keeps the layout exactly as you see it.

Can I add my clinic logo?

Not on this template. The FindClinica app prints your logo faintly behind every prescription it makes; set it as your profile picture in the app.

Is my PMDC number required on a prescription?

We couldn't find a rule that makes it compulsory on the prescription itself (PMDC asks for your registration certificate to be displayed where you practise), but patients and pharmacies expect to see it.

Can I remove the small findclinica.net line at the bottom?

Yes. Untick it in the form and it disappears from the pad.
