Paper register, Excel or clinic software: what should a small clinic use?
We make clinic software, so read this knowing that. We'd still rather you pick the right tool than the most expensive one, and for some clinics a register is honestly fine.
Below are real prices from Pakistani shops and Microsoft's Pakistan store, checked in October 2026, and a plain answer to when each option stops working.

What each option costs
Prices are in rupees and change often, so treat them as a rough guide. The links are at the bottom of the page.
| Paper register | Excel or Google Sheets | Clinic software (FindClinica) | |
|---|---|---|---|
| Getting started | A 400-page register is about Rs 485. Card files are Rs 1,300 to 2,900 for 50. A steel almirah for the files is Rs 30,000 to 50,000. | A computer you already have, or a used laptop for about Rs 60,000. Excel also runs free on phones for basic editing. | The Android phone you already have. A reception computer is optional. |
| Every month | New registers, files and prescription pads (Rs 1,200 to 8,500 per 1,000 sheets, depending on the pad). | Google Sheets is free. Microsoft 365 Personal is Rs 2,299 a month or Rs 22,999 a year. | Free, with no limit on patients. Pro is PKR 2,000 a month. |
| Finding an old patient | Turn pages, usually by date | Search works, if the sheet was kept tidy | Type part of the name or phone number |
| Instalments and dues | Added up by hand | Formulas, if someone sets them up and nobody breaks them | Worked out per visit and per patient |
| Two people at once | One register, one person | Shared sheets get overwritten | Your phone and the reception computer on the same records |
| If it's lost | A leak, a fire or a borrowed register and it's gone | Depends on where the file was saved | Synced to the server and backed up daily |
| Prescriptions | Handwritten pads | Not really | Printed, or sent on WhatsApp |

When a register is good enough
If you are just opening, see a handful of patients a day, rarely need an old file and do all the paperwork yourself, a register works. It needs no power, no internet and no training.
If you stay on paper, use a pre-printed format for notes and prescriptions. In a Peshawar study, prescriptions with no legibility problems went from 76% to 94% once doctors used a printed form, which is a big gain for almost no cost.
When Excel is good enough
If you are comfortable with spreadsheets, work alone, and mostly need a list of names, numbers and fees, Excel or Google Sheets will do. Google Sheets is free with a Google account, and the Excel app is free for basic editing on phones and small tablets.
The trouble starts when a sheet has to hold visits. One row per patient doesn't fit a patient who comes eleven times; one row per visit makes the history hard to read. Formulas get broken, copies end up on WhatsApp, and there is no record of who changed what.
When clinic software starts to pay for itself
Software earns its place when you have repeat patients with follow-ups and instalments, more than one doctor or a receptionist, appointments to remind people about, prescriptions to send, or registers to keep for the healthcare commission.
The real cost of paper is time and missed information. In a large study of primary care clinics, information was missing in 13.6% of visits, and doctors felt that in more than half of those cases it could delay care. Clinicians with complete electronic records were much less likely to be missing it.
What about load-shedding and the internet?
It matters more in some cities than others. In NEPRA's latest yearly report, distribution companies reported average daily load-shedding of about 10 hours in QESCO's area, 8 in HESCO's, 5.5 in PESCO's and about 5 in K-Electric's, while Lahore, Islamabad, Faisalabad and Gujranwala reported very little.
Paper doesn't care. Excel on a laptop with a charged battery is fine too. FindClinica keeps working on the phone with no internet and syncs later; the one thing it can't do offline is send a prescription, because the prescription image is made on our server.
Privacy is a problem with all three
Pakistan's Personal Data Protection Bill still hadn't become law in 2026, so there is no specific rule today about where patient records may be kept. The practical risks are the same either way: a register anyone can read, a laptop without a password, a sheet forwarded to the wrong WhatsApp group.
Software doesn't make those risks disappear, but it does put the records behind one account with a known list of devices.
If you do switch
Don't copy the old registers in. Start new patients in the app from a Monday, add old ones when they come back, and keep the registers on the shelf. There is no import from Excel yet, so the same goes for a spreadsheet. How the patient record app works.
Common questions
Is Google Sheets safe for patient data?
It is as safe as the Google account and the people it is shared with. Use a strong password and two-step sign-in, and don't share the sheet with anyone who doesn't need it.
Can FindClinica import my Excel sheet?
Not yet. Most clinics add patients as they come in, which also cleans out the old entries nobody needs.
What's the cheapest way to try software?
Install FindClinica on your Android phone and use the free plan, which has no limit on patients. You don't need a computer to start, and the desktop app is free when you want one.
Do I need a printer?
No. Most patients are happy to get their prescription on WhatsApp. If you want to print, a basic ink-tank printer is around Rs 32,000 and a small laser printer around Rs 42,000 to 55,000.
